Admission Criteria

Drugs:   Hard drugs and cannabis

 

Applicants must be “clean” for at least three months, and have undergone formal rehabilitation before admission.

Applicants must be willing to accept help to remain “clean” and there must be no present “recreational use.”

 

Alcohol

 

Applicants must be “dry” for at least six months at time of application.  Past abuse of alcohol is not a disqualifying factor, but applicants must have undergone some formal rehabilitation to help deal with the problem.

 

Violent or Aggressive behaviour

 

There must be no current or habitual violent or aggressive behaviour.

 

The expectation is that patterns of violent or aggressive behaviour will have been broken for at least one year - it is a condition of entry that this period has been achieved.

 

Learning difficulties

 

2 Care also works with people who have learning disabilities.

 

Physical Disabilities

 

People who require nursing or personal care may not be accepted, and this may include individuals with certain mobility issues.

Age

 

Applicants must be over 18 at time of admission.

 

Risk Assessment

 

We will require the most recent CPA and risk assessment in order to help us match applicant’s needs to our service provision.

 

Referral Sources

 

Referral is usually through social services departments or health authorities.

 

Application Procedure

 

After an initial enquiry a brochure and other relevant information will be issued.  Upon referral, and after receipt of the completed application form (please go to the ‘Contact us’ section to download our current application form), applicants are invited to an informal visit to 2 Care, so we can show what services we offer and the support we provide. Service users are invited to be accompanied by their social worker, advocate and/or family member.

 

If all parties wish the referral to go further, 2 Care may request specific background information, including recent C.P.A.s and Risk Assessments to assist in making a final decision.

 

If agreement is reached, a six week trial will be put in place.

2 Care requests written confirmation in advance that full funding is agreed for the duration of the six weeks, and also advanced written confirmation that the relevant referral authority remains responsible for providing the individual with a place of safety if the trial breaks down for any reason.
 
The six week trial represents a mutual day to day assessment, following which there is a Review before a long term placement would be confirmed.

 

The service user will then be asked to sign a tenancy agreement.

 

Psychiatric and Social-Work Support

 

Service users are expected by 2 Care to be overseen by their referring psychiatrist until any handing–over process is completed by the local CMHT and referring body, and 2 Care notified in writing.

 

Ideally this should be achieved before placement is confirmed and all referral teams should make this a priority.

Social workers and service users are expected to maintain regular contact, with at least annual CPA/risk assessment reviews.

 

Staff will ensure that service users are registered with a local GP and where possible, a local dentist.

In the event of a service user being admitted to hospital, their place at 2 Care can be retained when satisfactory financial arrangements can be reached.

 

It is expected that staff will be involved in review meetings and plans for the discharge of the service user back to 2 Care.

 

Record Keeping and Confidentiality

 

The way 2 Care records information and our commitment to service user confidentiality can be read in the relevant Policy and Procedures enclosed.

 

Rules and Expectations

 

No harassment on the grounds of race, colour, religion, sex, sexual orientation or disability; no violent or aggressive behaviour to staff or another service user at 2 Care.

 

Smoking is not permitted in bedrooms, and only in designated areas.

 

Alcohol is not permitted on the 2 Care site. However, exceptions can be made on special occasions with prior negotiations with 2 Care management

 

Use and possession of illegal drugs is not permitted.

 

Service users are expected to take their medication as prescribed.

 

Service users will be required to sign a tenancy agreement which will form part of the contract between 2 Care and the service user.

 

For security reasons service users are required to inform 2 Care of any visitors they have on 2 Care premises and that they sign in and out using the Visitors book.

 

Service users are requested for security reasons not to enter another service user’s room, unless prior permission is given by the 2 Care management team.

 

If it is felt that the safety of the community is severely threatened by the behaviour of a service user, 24 hours notice may be given.

 

Equal Opportunities/Data Protection

Charges are based on an assessment of individual need.

 

What the charges cover:-

 

Accommodation

 

Covers full board and lodging and access to on-site facilities and activities.

 

Support

 

2 Care provides a housing related support service which aims to develop or sustain an individual’s capacity to live as independently as possible in the accommodation provided.

 

Referring authority may, subject to assessment, have to make additional payments to cover the cost of:

 

      • General social/personal care
      • Specialist counselling
      • Or any service that statutory agencies have a duty to provide or fund.
